SUPERVISOR

SUPERVISOR - (NEW HAVEN)  needed for assessment and training in: self-care; medication management; task completion; communication and interpersonal skills; socialization, sensory/motor skills; mobility and community transportation skills; problem solving skills; and, money management and ability to maintain a household for people with Acquired Brain Injury (ABI).  High School diploma or general education degree (GED) with two years of relevant experience is required; two years working with individuals with TBI/ABI and 2 years of supervisory experience.  Must be at least 18 years old and have a valid drivers' license, reliable transportation with valid registration and insurance.

-Ensure all consumer related communications are reported to all in-house staff members.

-Maintain ongoing knowledge and understanding of Consumer’s individual plan of care.

-Follow crisis intervention plans and ensure that all staff are aware of processes.

-Ensure implementation of behavior programs and recorded data.

-Attend all team / staff / management meetings.

-Ensure all goals are met by taking notes in team meetings, creating any necessary documents and transmitting information to all house staff.

-Responsible for training new staff according to company policy and all consumer protocols

-Provide guidance and direction to all of consumer’s staff, keeping Program Manager informed of staff performance deficiencies.

-Be first point of contact for call-outs and vacation requests.

-Maintain staff book ensuring documentation of all program reporting requirements, i.e. incident reports, logs, diet charts, weight logs, physician consult sheets, time off requests, etc.

-Responsible for meal planning and ensure implementation.

-Ensure household has sufficient supplies.

-Ensure the protection of resident’s rights.

-Ensure consumer’s budget is in place and necessary documentation is completed by all staff members.

-Ensure the health & safety of the environment.

-Participate as member of the Interdisciplinary Team.

-Ensure scheduled appointments are being kept and all required documentation is being submitted.

-Ensure weekly reports and all required documents are ready for pickup on Monday before 9:00 am.

SKILLS:

•       Ability to speak and write clearly and effectively in all situations asking for clarification when needed.

•       Ability to read and interpret complex information and instructions.

•       Ability to evaluate, document and report consumer progress.

•       Computer literate.
