Assists the resident through assessment, referral and direct services to move toward independent living status. Monitor and supervise client activity regarding adherence to service plans. B.A. Degree + one year exp. administering case management services; or A.S. degree in human services + three years exp. in community based case management services. Bilingual (English/Spanish) a plus. Valid CT Driver's License required. Must be proficient in Microsoft Word, Excel and Outlook. $35,000/yr.
