Job Title: SOCIAL WORKER

Duties:

Full Time position, reporting to the Transition Manager.

The Social Worker will provide leadership, design and assist Leeway in building a comprehensive and interdisciplinary behavioral health program for residents living with HIV/AIDS.  The program will benefit the residents who are experiencing medical, cognitive, affective, behavioral, and/or psychosocial challenges related to multiple life stressors including addictions, substance abuse or mental health issues, and to facilitate a successful community placement upon discharge.

Completes assessments, benefit/entitlement applications, updates MDS, and schedules resident care plan meetings.  Provides individual therapeutic counseling and facilitates Recovery and Wellness groups.  Assists with transporting residents as needed. Assist with recreation activities as needed.  Performs other duties as assigned.

Skills and Experience:
Master’s Degree in Social Work or Licensed Master’s Degree in Social Work with two years’ experience preferred.  Must have experience in working with the homeless population with mental health and/or substance abuse diagnoses.

Long term care experience (Skilled Nursing Facility) required

Salary: Commensurate with experience starting at $50,000.
