RN Charge Nurse  All Shifts

Leeway, Inc.- New Haven, CT

30 bed SNF in New haven, CT is seeking a Charge Nurse.  One year experience in med/surg is a must. 

Knowledge of infection control policies and procedures is a must.  IV certification required. 

The appropriate candidate will report to the DNS and be responsible for  instructing and monitoring the care of the patients and giving directions to the LPN & CNA.

Duties will include directing the admission, discharge and general flow of patients, and assisting nurses and support staff for resident care.

  Must have experience in assessing patients changes in baseline condition and informing the physician of changes in assessment when appropriate. 

Experience with psychiatric/ dual diagnosis patients is a plus.  Must be proficient with infection control policies and procedures. 

The eligible candidate must be able to respond to emergencies; assess the condition of the resident; decide on the type of medical attention required; call appropriate ambulance service if needed; notify family when resident is transferred or admitted to the hospital and notify the physician when a change in condition occurs.  IV certification and BLS certification required. 

Must be compassionate, empathetic to patients when providing care.  Must have experience with nursing assessments; medication administration; Nursing care plan development and implementation; Wound care/ dressing changes; Catheterization; g/j tube feeding; Colostomy/ileostomy care; O2 administration; Evidence Based Practice; Experience using SBAR; Ability to establish trusting relationships with patients.
