Ambulatory Clinic Site Manager

 

Job Title:  Ambulatory Clinic Site Manager   

Reports to:  Chief Operating Officer         

 

A.  Job Purpose:  To provide leadership to the site staff and to assist clinicians and clinical assistants in providing care to the patients of the Fair Haven Community Health Center consistent with FHCHC policies and mission. Primary oversight of the operations of a multidiscipline ambulatory center to maintain high quality services and efficient functioning to meet the goals of a patient centered medical home.

 

B. Specific Duties and Responsibilities

  • Responsible for the day to day operations of the site.
  • Oversees the flow and distribution of patients, optimizing resources and clinical care in all disciplines.
  • Manages Patient Access and productivity across the disciplines.
  • Provides direct support to the reception area and maintains a calm efficient work environment.
  • Collaborates with the Director of Patient Access for competency testing and performing annual reviews of Patient Access Representatives.
  • Manages daily staffing levels and provides hands on leadership throughout the day.
  • Directly supervises clinical support staff including hiring, training and development.
  • Collaborates with the Nursing Administrator for competency testing and performing annual reviews of the clinical support staff
  • Ensures the functioning and availability of medical supplies and equipment.
  • Oversees site reporting, resource management, and quality assurance requirements as established by the organization and regulatory bodies.
  • Engages in performance improvement activities and acts as an agent of positive change by looking for innovative ways to improve processes and advance organizational excellence.
  • Provides direct patient care as needed to facilitate optimal patient flow/care, if licensed.
  • Demonstrates the principles of good management and communication skills
  • Ensures patient confidentiality and safety.
  • Adheres to all Health, Safety and Infection Control Policies and Procedures.
  • Provides excellent customer service and culturally competent care.
  • Positively represents FHCHC to internal and external clients.
  • Performs other necessary duties as required by FHCHC to achieve the goal of providing excellent health care in a Patient Centered Medical Home.

 

 

C.     Qualifications

1. Knowledge

  • Ability to work with all levels of staff, entry level to executive, clinical and non-clinical.

  • Excellent time management skills, attention to detail, and critical thinking ability.

  • Demonstrated ability to identify and solve problems collaboratively.

  • Flexible, able to successfully manage competing demands and changing priorities while staying calm under pressure.

  • Able to work with minimal direction, demonstrate initiative and able to be self directed as well as an effective team player.

  • Demonstrated ability to lead and motivate a team.

  • Excellent written, electronic and oral communication skills.

  • Ability to relate with warmth and effectiveness to patients and providers.

  • Skilled in reporting, presenting and meeting facilitation.

  • Experience and familiarity with Joint Commission standards of excellence, Patient Centered Medical Home Model and interdisciplinary care.

2.Technical Qualifications:

  • Bachelor’s Degree

  • Clinical background, Registered Nurse preferred with current CT License

  • Minimum 3 years Supervisory or Management experience

  • Minimum of 5 years Clinical experience; at least one year in a fast paced primary care setting preferred

  • Competency with initial basic medical assessment of children and adults, if licensed.

  • CPR Certified, Basic Life Support (BLS)

  • Oral and written proficiency in English required.  Bi-lingual with fluency in Spanish preferred.

  • Strong computer skills including Microsoft Office, including Word and Excel.

  • Knowledge of Epic a plus.

 

D.     Direction of Others: Supervisor of non-clinical and clinical support  staff

 

       E. OSHA STATUS—CATEGORY I-Increased Risk Position
