Social Worker Excellent Long Term Care facility is seeking a qualified and compassionate social worker. Qualified candidate will assist in planning, organizing, implementing and evaluating the overall operation of the Social Services Department in accordance with federal and state standards to assure that the individual emotional and social needs of short term rehabilitative, long term, and memory care residents are met/maintained. Requirements include: MSW; experience in discharge planning; knowledge of state/fed nursing home regulations and services for the aged; Social Work experience in a nursing home environment.
