FT CASE MANAGER FOR SHELTER IN MERIDEN provide case management services to clients assigned. Assist the resident through assessment, referral and direct services to move toward indendent living status. Monitor and supervise client activity regarding adherence to service plans. B.A. degree + one year experience administering case management services or A.S. degree in human services + three years experience in community based case management services. Bilingual (English & Spanish) a plus. $16.83/hour, full-time, 40 hours/week, Monday-Friday, 8:00 am to 5:00 pm.
