HOUSING CASE MANAGER

The Housing Case Manager will provide case management services for those tenants who reside in Leeway Supportive Housing.

The Housing Case Manager is responsible for assisting tenants in maintaining permanent housing; empowering tenants to live independently; maintain their health; learn independent living skills needed for positive community living; and to increase time spent in positive social behaviors.

The Housing Case Manager will comply with the case management services and documentation standards set forth by the funder. A Bachelor’s degree from a four-year college or university in a related field or equivalent work experience is preferred.

The qualified candidate must have experience working with individuals with mental health and substance use issues and must possess a valid CT Driver’s License. Salary is dependent upon experience.
