Case manager
Seeking a full time case manager for a supportive living program in Norwalk, CT.
Provides rehabilitative mental health and substance abuse services to assist individuals to achieve and maintain the highest degree of independent functioning in the community. Works collaboratively to provide intensive rehabilitative community supports, crisis intervention, group and psycho-education, case management activities, person centered goal development.
